I create a new request form for a holiday lighting contest we are holding in the community. How to get that form to show up on the website and homeowner log in?

Go to Voting-Surveys page and then forms. From here you will see a heading called "copy to another location". Click on this arrow for the form and select copy to websites forms. Make sure the form has the setting called "survey display mode" set to editable. If this setting is set to read-only then homeowners will not be able to fill out the form on the website. You will also need to make sure the forms widget is set up on the website. You can make sure that a forms widget is setup by going to the tools page and then website page. From here click on the page you want the forms widget to show up on and add the forms widget to the page you want.
